Application for Admission to Nakile Home for Special Care
If you are interested in seeking admission to Nakile Home for Special
Care, you will need to apply through Home Care Nova Scotia.
All applicants must submit a medical assessment, a social history and a
complete financial assessment. Completed applications are
then submitted to a provincial classification officer for approval.
Some of the provincial guidelines for admission to a long-term care
facility include:
-
The applicant must have a
medical condition that warrants admission. A person will not
qualify for admission on age alone.
-
The applicant must have a
stable medical condition. They must not require
frequent medical attention.
-
The applicant must not be disruptive or cause a potential threat to the
residents of the Home.
A Care Coordinator with Home Care Nova Scotia will review the financial
obligations with the family and applicant prior to admission to
Nakile. Be assured that these forms remain strictly confidential and are
utilized for admissions procedures only.
The cost of living at Nakile is determined by Home Care Nova Scotia.
On-going costs not covered in the per diem rate will remain the applicant's
responsibility, ie: vision care, dental, some medications and special equipment. |
